Glenwood Resource Center

The mission of Glenwood Resource Center (GRC) is to improve the quality of life for individuals with disabilities by providing comprehensive service in partnership with customers to accomplish individualized team developed goals.  Discharge planning begins at admission; GRC partners with providers of community-based services to assure a bridge to the community of preference once an individual has stabilized and gained the skills necessary for success.

Who is eligible to receive services:
Children and adults with mental retardation and other related disabilities from throughout the state of Iowa are eligible.  Each type of service has specific eligibility requirements.

How to apply for services or refer someone to this facility:
The county Central Point of Coordination Administrator can assist with accessing services for adults.  The county Department of Human Services office or case manager will assist with locating services for children.  If a decision is made to apply for GRC ICF/MR services, the application process begins by calling the Department of Human Service Help Desk at 866-347-7782.

Structure of the organization:

Glenwood Resource Center is operated by the Iowa Department of Human Services, under the interim directorship of Sally Titus and under the leadership of Sally Titus, Deputy Director for Field Operations.  The facility’s superintendent is Thomas J. Hoogestraat.

 

Interdisciplinary teams are led by Qualified Mental Retardation Professionals (QMRPs) in an integrated approach to service provision.  Team members include the individual, the individual's family/guardian, local county or DHS staff, direct support professionals, psychologist, psychology assistant, social worker, occupational therapist, speech/language pathologist, physical therapist, audiologist, vocational/day program specialist, leisure specialist, dietitian, nurse, physician, pharmacist, psychiatrist, and dental assistant.  Specialty medical services are offered by community providers through on-campus clinics and include: podiatry, surgery, otolaryngology (ENT), gynecology, and optometry.

 

Services are person-centered and strengths-based.  GRC’s goal is for any person who chooses to transition to a community based-setting to have that opportunity.

 

Services provided:

 

Intermediate Care Facility/Mental Retardation (ICF/MR) Residential Services

Glenwood Resource Center (GRC) works with the community to explore all reasonable and appropriate options before a person comes to live on the campus.  Generally, people come to GRC because they need the specialized expertise available due to behavioral or medical conditions that are complex and chronic.

 

Individuals, working as part of their Interdisciplinary Team, help to develop their own person-centered Individual Support Plans, which are designed to meet their specific support needs, preferences and goals.  Professional and direct support staff supports the person in meeting these goals.  All services offered on the campus meet rigorous federal standards for active treatment, safety, and health.

 

SPECIALTY SERVICES

Assistive Technology

When individuals with disabilities can control their environment, they can participate more fully and independently in daily living.  Customized devices are tailored to meet each individual’s needs.

 

Physical Nutrition Management Team

This team evaluates, treats and manages dysphagia (feeding disorders) and oral motor dysfunction.  The team considers positioning, nutrition, and all activities that involve swallowing.  The team can provide consultation and training for individuals with disabilities and providers throughout the state.

 

Behavioral Services

Behavioral Services includes highly structured assessment methods to generate efficient plans that families and service providers can use to bring about positive changes in the lives of persons presenting challenging behaviors.  Services include:

            Leadership and training in the use of Applied Behavior Analysis;

            Functional Analyses of Behavior;

            Development and implementation of behavior support plans consistent with the results of Functional Analysis of Behavior;

            Ongoing technical assistance to those receiving GRC services; and

          A hierarchy of behavioral interventions to address the challenging behaviors of individuals.

 

Positioning Therapy

Positioning therapy services focus on improving health and preventing complications associated with body position in various settings.  Specialized wheelchair adaptations, adaptive equipment and bed positioning are some of the supports offered through positioning therapy services.

 

Hydrotherapy

A hydrotherapy pool is available on the GRC Campus.  This state-of-the-art heated pool has a moveable floor and an access ramp that allow for handicapped accessibility and adjustment of water depth.  Hydrotherapy is known to promote health, encourage a sense of well being and decrease pain associated with various ailments.

 

FAMILY SUPPORT AND OUTREACH SERVICES

Family Support and Outreach Services are designed to assist persons with disabilities, their families, and providers of community-based services in supporting individuals in their communities.  For assistance, please call the GRC Social Work Administrator.  Early intervention is often the most successful. 

 

Consultations

Consultations are provided by staff with expertise in the particular area of need.  Services are provided where the individual lives and works or goes to school; if the customer prefers, service can also be provided on the GRC campus.

 

Time Limited Assessments

Time-Limited Assessments (TLAs) are offered for a period of up to 30 days, on the Glenwood campus, as part of the ICF/MR program.  The goal of the Time Limited Assessment is to determine effective treatment options that can be used to support an individual in the community.  Time-Limited Assessments include assessments, evaluations and recommendations from all disciplines available at GRC with a focus on addressing specific challenges experienced by the individual.  Recommendations and follow-up services are offered when the consumer returns to the community.

 

Individual and Facility-Based Respite Care

Respite care supports family unity and reduces the need for out-of-home placement.  Respite care offers a period of care for the individual during which families can rejuvenate, relax and attend to other needs.  GRC offers respite services both on campus and in the community/family home.

 

In-Home Services
Supported Community Living services assist people with disabilities to gain skills and access resources necessary to maintain independence in their homes and communities.  Residential Treatment Technicians support people as they work on goals related to home skills; personal care skills, community access and other independence related needs.  Support is available on an hourly or daily basis.  Hourly SCL services are provided in the individual’s home or community.  Daily SCL services are provided to individuals who have transitioned from the Resource Center into their own homes in the Glenwood community.
 
Supported Employment
Supported Employment services are offered to individuals receiving daily SCL support in their homes through the HCBS Waiver program.  These supports assist individuals with disabilities in gaining meaningful employment in integrated settings.  Guidance is provided to teach the skills necessary to attain and maintain employment while offering supervision and support.
 
Eligibility and Funding

Consultations may be provided to those in need, as resources are available and service needs may be addressed by the expertise available among professionals employed at GRC. 
 

Time-limited Assessments are available, as GRC’s resources allow, for individuals who have specific assessment needs that can be met within the parameters of the TLA process and who meet funding requirements.  Services are funded as a part of ICF/MR services through Medicaid and/or county funding.

 

Respite care, Supported Community Living and Supported Employment are services offered through the Home and Community-Based (HCBS) Waiver programs.  GRC may provide services and access funding for those approved for HCBS Waiver eligibility in the following areas:

          Mental Retardation Waiver

          Ill and Handicapped Waiver

          Brain Injury Waiver

          Children’s Mental Health Waiver

          Elderly Waiver

People served:
The Glenwood Resource Center campus is located in Glenwood, Iowa and serves people from a 52-county catchment area within the state.

Residential services (ICF/MR) are provided on the campus for approximately 325 people diagnosed with mental retardation.

 

HCBS Waiver services are currently provided to approximately 51 people in their homes and communities.  Services are offered within a 50-mile radius of GRC.

Time Limited Assessment is provided, as space allows on the GRC campus, within the ICF/MR program, for up to 30 days for persons with mental retardation from throughout the state.
